Job Description

The Campus Community Health Worker will represent Ohio University and work closely with undergraduate and graduate students on and off-campus. The CHW will increase engagement and serve as a bridge between students, campus and community resources, and healthcare services and will support student health and wellness through education, goal-setting, and health outreach. The CHW engagement strategies will be informed by identifying and addressing population needs based on developing and implementing a social determinants assessment.

This is a term position through June 30, 2025. Potential for extension dependent on funding.
  • Develop and deliver an assessment strategy for individuals and/or specific focus populations on social determinants to identify health needs. Provide follow up support and resource utilization to individuals who indicate interest, or need for care. Create, administer, and analyze assessment instrumentation and provide an executive summary with accompanying action plan to implement relevant, impactful intervention strategies to meet or support student health needs and bridge gaps where necessary. Develop tracking system for resource and referral utilization.
  • Identify and collaborate with on-campus and community-based efforts to ensure comprehensive promotion of knowledge and understanding regarding resources available to student body based on social determinant assessment findings. Develop and disseminate messaging that educates campus staff and students support resources.
  • Initiate and develop system to document engagement of CHW in the care, support, and follow-up across campus and community settings.
  • Integrate into campus-based services, public health emergency preparedness and planning processes, and care teams to support student health.
Department & Student Affairs: Actively contributes as a staff member of the Department of Well-Being & Recreation and the Division of Student Affairs that upholds the framework of equity and social justice and the personal well-being of the university community. Provides leadership, promotes collaboration, and facilitates effective communication across all department, divisional, university and community partners. Maintains current knowledge of the University, its environment, purpose, and affairs to be prepared to respond to appropriate issues. Actively participates in professional development opportunities and maintains knowledge of current trends and best practices within area of responsibility. Advocates and promotes diversity and inclusivity through role and responsibilities in support of department, division, and university initiatives. Infuses socially just practices into the work, making sure programming and trainings are inclusive and accessible to all students. Performs other duties as assigned.

Minimum Qualifications

High School Diploma or GED

Preferred Qualifications

CHW Certified
Associate's degree, Bachelor's degree, or vocational or technical school degree in Public Health, Social Work, Nursing, or other health related field.
Experience working within a higher education environment and/or university student population.
