The Community Health Worker position is a grant funded position that is responsible for increasing enrollment in the Heritage College Primary Care Breast & Cervical Cancer Program. This position will require developing and increasing community partnerships in and around nine Appalachian counties to promote and increase enrollment in the Breast and Cervical Cancer Program.

Primary Duties:
  • The successful candidate will be expected to increase enrollment into the Breast & Cervical Cancer Program through community engagement and promotion of the program through the development of and participation in community activities and programs.
  • Increasing visibility of the program through outreach and expansion of community/agency partnerships will be essential.
  • Identifying additional partnerships that will result in an increase of referrals to the Breast & Cervical Cancer Program.
  • Regular reporting on activities, events, and promotions.
  • Other duties as assigned.
Minimum Qualifications
  • High School diploma or GED required
  • Valid Drivers License or reliable transportation for outreach to the community
  • 1+years of related work experience
Preferred Qualifications
  • Associate's Degree Preferred
  • Community Health Worker Certification
